Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless patient care transitions to optimize outcomes
Master complex discharge planning for safe patient returns home
Collaborate with multidisciplinary teams to advocate for patient needs
Utilize EMR systems like Meditech and Midas to track progress efficiently
Ensure high-quality care delivery in a fast-paced hospital setting

Qualifications

Active RN license in good standing
Minimum 1 year of case management experience
Proficiency in Midas and Meditech systems required
Experience with complex discharge planning preferred

Education

RN degree from an accredited nursing program

Knowledge & Skills

Expertise in RN case management practices
Strong skills in Midas and Meditech EMR systems
Ability to handle complex discharge planning
Excellent collaboration with RN CM and SW CM teams
