Roles & Responsibilities

• Perform thorough patient assessments and implement individualized care plans
• Administer medications safely and document administration accurately
• Provide wound VAC care, wound management, and other advanced treatments
• Manage gastrostomy (G-tube) care and urinary catheter care with confidence
• Deliver nebulizer treatments and CPAP support as ordered
• Assist residents with activities of daily living (ADLs) to preserve dignity and independence
• Complete admissions and discharges, coordinating with interdisciplinary team members
• Communicate pertinent clinical information to MD/NP and escalate concerns promptly
• Maintain clear, timely documentation and contribute to a patient-centered care environment

Qualifications

• Active Hawaii RN license required by start of assignment
• Current Registered Nurse (RN) with demonstrated Long Term Care (LTC) experience
• Comfortable with medication administration, patient assessment, and ADL assistance
• Experience with wound VAC care, G-tube management, urinary catheter care, nebulizer treatments, and CPAP
• Ability to work rotating 8-hour shifts including nights, weekends, and holidays
• Strong communication skills for collaborating with MDs, NPs, and interdisciplinary teams

Knowledge & Skills

• Skilled in patient assessment and documentation
• Proficient with medication administration and safe handling
• Clinical experience in wound care and use of wound VAC systems
• Gastrostomy (G-tube) and urinary catheter care competence
• Familiarity with respiratory supports including nebulizers and CPAP
• Able to manage admissions, discharges, and care transitions
• Comfortable providing ADL assistance and person-centered care
