Roles & Responsibilities

• Provide compassionate end-of-life care to patients and families in their homes
• Deliver expert pain and symptom management
• Collaborate closely with an interdisciplinary team to optimize quality of life
• Manage patient caseload and complete Oasis documentation
• Coordinate with local physicians and community providers

Qualifications

• At least 1 year of nursing experience
• Active Vermont RN license (must be currently active)
• BLS/BCLS certification required
• OASIS experience is a must
• Home Health and Hospice experience required
