Roles & Responsibilities
- Conduct comprehensive initial assessments for patients across all hospital departments
- Perform utilization review using Interqual criteria to ensure appropriate level of care
- Develop and execute discharge plans coordinating with patients, families, and care teams
- Manage a caseload of 15-20 patients per day, addressing diverse payer sources and complex social needs
- Collaborate with hospitalists, intensivists, and multidisciplinary teams to optimize patient outcomes
- Document and track patient progress in Cerner EMR system
Qualifications
- Active and unrestricted Registered Nurse (RN) license in California
- Current BLS certification
- Minimum 1 year of recent case management or utilization review experience in a Dignity Health/CommonSpirit facility
- Proficiency with Interqual and Cerner systems
- Strong clinical assessment and critical thinking skills
Knowledge & Skills
- Working knowledge of Interqual criteria for utilization review
- Expertise in discharge planning and care coordination across inpatient settings
- Excellent collaboration and teamwork abilities
- Effective communication with patients, families, and healthcare providers
- Ability to prioritize and reprioritize tasks in a fast-paced environment
- Familiarity with diverse payer sources including uninsured and homeless populations
