Roles & Responsibilities

• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and Intermediate Care Facility (ICF)
• Perform evaluations, reassessments, and treatments consistent with your licensure and scope of practice
• Develop and implement individualized treatment plans in collaboration with the supervising therapist and interdisciplinary team
• Apply evidence-based therapeutic interventions to maximize function, mobility, and quality of life
• Educate residents, families, and caregivers on treatment plans and progress
• Complete accurate, timely documentation per facility, state, and federal requirements
• Supervise therapy assistants as applicable
• Be available to work Hawaii state and federal holidays as needed

Qualifications

• Minimum 2 years of experience in a SNF/ICF setting preferred
• Current Basic Life Support (BLS) certification
• Active State of Hawaii Physical Therapy Assistant license in good standing

• Rehabilitation therapy techniques in long-term care
• Skilled Nursing Facility (SNF) and Intermediate Care Facility (ICF) protocols
• EMR documentation systems (Net Health Rehab Optima & Epic)
• Interdisciplinary collaboration and patient education
• Evidence-based practice and clinical reasoning
