Roles & Responsibilities

• Provide comprehensive physical therapy evaluations and develop personalized treatment plans for skilled nursing residents
• Implement therapeutic activities to improve mobility, strength, and functional independence
• Collaborate with nurses, physicians, and other therapists to optimize each patient's recovery
• Educate residents and their families on home exercise programs, fall prevention, and adaptive equipment
• Document patient progress accurately and maintain compliance with regulatory standards
• Participate in care plan meetings and contribute to discharge planning

Qualifications

• Current and active Physical Therapist (PT) license in California
• New graduates are encouraged to apply - all experience levels welcome
• Experience in skilled nursing or geriatric rehabilitation is a plus, but not required
• Strong clinical reasoning and manual therapy skills
• Compassionate and patient-centered approach to care

Education

• Doctor of Physical Therapy (DPT) degree from an accredited program

Knowledge & Skills

• Doctorate of Physical Therapy (DPT) preparation
• Expertise in therapeutic exercises and functional mobility training
• Knowledge of SNF regulations and documentation standards
• Excellent interpersonal and communication skills
• Ability to work effectively within an interdisciplinary team
