Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans that promote recovery and mobility
• Provide skilled therapeutic interventions and progress care to meet functional goals
• Document patient progress clearly and timely to ensure continuity of care
• Collaborate with nurses, occupational therapists, physicians and family members for holistic care
• Educate patients and caregivers on home exercise programs and safety strategies
• Mentor and support PTA/assistive staff (when applicable) to maximize patient outcomes

Qualifications

• Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in good standing — NH licensure or ability to obtain
• Active BLS/CPR certification (or willingness to obtain prior to start)
• Strong clinical assessment and manual therapy skills relevant to rehabilitation
• Clear, compassionate communicator with excellent teamwork skills
• Previous inpatient or skilled rehab experience preferred

Education

• Physical Therapist: Doctor of Physical Therapy (DPT) or equivalent
• Physical Therapist Assistant: Associate degree from an accredited PTA program

Knowledge & Skills

• Skilled in patient assessment, treatment planning and progress measurement
• Proficient with therapeutic modalities and evidence-based rehab techniques
• Comfortable with documentation systems and regulatory compliance
• Ability to safely transfer and mobilize patients and train caregivers
• Strong time management and prioritization in a busy clinical setting
