Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy services including start of care, recertifications, and evaluations
- Deliver personalized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
- Conduct daily home health visits to support patient recovery and independence
- Collaborate with clinical managers and educators to ensure quality care delivery
- Document patient progress and adjust treatment approaches as needed
Qualifications
- Current Physical Therapist license required
- BLS certification required
- Home health experience preferred but not required
- No new grads - experienced clinicians preferred
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Home health physical therapy expertise
- Strong assessment and evaluation skills
- Excellent communication and patient education abilities
- Comfortable working independently in patient homes
- Proficiency with electronic documentation systems
