Roles & Responsibilities
- Perform comprehensive OASIS assessments for new patients
- Conduct Start of Care evaluations across four rural counties
- Develop personalized treatment plans for elderly patients recovering from falls and fractures
- Provide skilled physical therapy interventions in patients' homes
- Document visits accurately using tablet-based tracking system
- Collaborate with interdisciplinary healthcare teams
Qualifications
- Current Physical Therapist license in Indiana
- Experience with OASIS assessments and Start of Care documentation
- Strong clinical skills in geriatric rehabilitation
- Reliable transportation for rural coverage area
