Roles & Responsibilities

• Conduct comprehensive patient assessments to develop individualized home health plans
• Implement therapeutic exercises and interventions to improve mobility and function
• Educate patients and families on safe techniques and home exercise programs
• Collaborate with physicians and interdisciplinary care team to optimize patient outcomes
• Document patient progress accurately and maintain compliance with home health regulations

Qualifications

• Active Physical Therapist (PT) license in the state of Idaho
• Home Health experience required
• BLS certification required
• Strong clinical and communication skills

Knowledge & Skills

• Excellent clinical assessment and problem-solving skills
• Clear communication with patients, families, and the care team
• Knowledge of home health safety protocols
• Ability to adapt treatment plans to a home environment
• Strong documentation skills
