Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across Yolo County
- Assess and evaluate patients' therapeutic needs while developing personalized care plans
- Guide patients and families through rehabilitation exercises and therapeutic appliance use
- Collaborate with physicians and healthcare teams to optimize patient outcomes
- Document progress and communicate changes in patient condition to ensure quality care
- Supervise home health aides and participate in case conferences for comprehensive care
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong oral and written communication skills with excellent organization abilities
- Valid Physical Therapy license for California practice
