Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide personalized physical therapy services to patients recovering from surgeries and falls in their homes
- Develop individualized care plans that help patients regain strength and independence
- Collaborate with home health and hospice teams to deliver compassionate, patient-centered care
- Document patient progress using HomeCare/HomeBase EMR
- Travel within a 60-mile radius to deliver therapy where patients need it most
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification required
- At least 1 year of physical therapy experience with previous home health background
- Valid driver's license and auto insurance
- Comfortable working independently without on-site manager supervision
Knowledge & Skills
- Home health physical therapy expertise
- Familiarity with HomeCare/HomeBase EMR
- Strong assessment and care-planning abilities
- Excellent communication and time-management skills
- Ability to cover a 60-mile radius around Valley City
