Location: Louisville, KY

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy evaluations and treatments in patients' homes
- Complete start of care, recertifications, and daily visits for diverse patient populations
- Develop personalized care plans for patients with CHF, orthopedic conditions, and pneumonia
- Collaborate with clinical managers and educators to ensure optimal patient outcomes
- Document patient progress and communicate effectively with the healthcare team
Qualifications
- Current Physical Therapist license in Kentucky
- BLS certification required
- Home health experience preferred but not required
- Valid driver's license and reliable transportation
- Strong communication and time management skills
Knowledge & Skills
- Home health physical therapy practices
- Patient assessment and care planning
- Electronic documentation systems
- Common diagnoses including CHF, orthopedic conditions, and pneumonia
- Independent decision-making in field settings
