Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Conduct start of care assessments, recertifications, and resumption of care visits
- Deliver 5-6 meaningful patient visits per day, making a real difference in recovery journeys
- Collaborate with clinical coordinators and fellow therapists for seamless patient care
- Complete OASIS documentation to ensure quality outcomes and compliance
- Perform occasional safety evaluations or teaching visits in hospice situations
Qualifications
- Current ND Physical Therapy License required
- BLS certification required
- Valid driver's license and auto insurance required
- Home Health experience and OASIS training preferred
- First time travelers welcome to apply
