Roles & Responsibilities
- Deliver personalized physical therapy in patients' homes
- Create customized treatment plans that promote independence
- Help patients regain mobility and improve quality of life
- Collaborate with interdisciplinary care teams
- Document progress and adjust care strategies
Qualifications
- Active California Physical Therapist license
- BLS certification
- Home health experience preferred
- Strong communication and time-management skills
Knowledge & Skills
- Patient assessment and care planning
- Therapeutic exercise techniques
- Home safety evaluation
- Electronic documentation systems
