Candidate Pay Rate: $59 per hour

Roles & Responsibilities
- Deliver personalized physical therapy care to patients in the comfort of their homes
- Design and implement treatment plans that help patients regain strength and mobility
- Collaborate with physicians and care teams to ensure seamless patient outcomes
- Educate patients and families on exercises and recovery techniques
- Document progress and adjust care plans to meet individual goals
Qualifications
- Active California Physical Therapist license required
- BLS certification required
- Home health experience preferred
- Strong communication and time-management skills

- Patient assessment and individualized care planning
- Effective documentation and EMR systems
