Roles & Responsibilities

• Provide skilled physical therapy assessments and individualized home health treatment plans
• Deliver hands-on care and therapeutic interventions that restore mobility, function, and independence
• Document patient progress accurately and timely in the electronic medical record (Epic preferred)
• Coordinate care with nurses, physicians, and interdisciplinary team members to optimize outcomes
• Complete Trinity specialty competency testing and skills checklists to maintain high standards
• Educate patients and caregivers on safe home exercise programs and fall prevention strategies
• Maintain compliance with required health screening and vaccine documentation

Qualifications

• BLS certification (required)
• License/certificate verification completed (per state requirements)
• Experience providing physical therapy in a home health setting (preferred)
• Comfortable with documentation workflows; Epic experience is a plus but not required
• Willingness to complete Trinity specialty competency testing and skills checklist
• Current health screenings and immunizations as listed in the job checklist

Knowledge & Skills

• Skilled in physical therapy assessment, treatment planning, and home-based interventions
• Knowledge of home safety assessment and fall-risk mitigation
• Strong clinical documentation skills and the ability to use electronic health records (Epic preferred)
• Excellent communication and patient/caregiver education skills
• Familiarity with infection control, vaccine requirements, and pre-employment health screenings
• Ability to work independently in patients' homes while coordinating with the care team
