Physical Therapist - Home Care
Location: Wall Township, NJ
Contract Type: Contract
Shift Type: Full-time, typically 40 hours/week; daytime visits with flexible start/end times based on patient caseload (Monday-Friday typical; exact times vary)
Hours per Week: 40
Candidate Pay Rate: $60- $70 per hour
Roles & Responsibilities
- Perform comprehensive patient evaluations in the home setting to assess mobility, strength, balance, and functional needs
- Develop and implement individualized treatment plans that promote independence and improve quality of life
- Provide therapeutic exercises, gait training, balance training, and manual therapy as clinically appropriate
- Educate patients and caregivers on home exercise programs and safety techniques to support long-term outcomes
- Monitor and document patient progress in compliance with agency and regulatory standards
- Communicate effectively with physicians, nurses, and case managers to coordinate care
- Participate in care plan meetings and contribute clinical expertise to interdisciplinary teams
Qualifications
- Active New Jersey Physical Therapist license required
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Current CPR/BLS certification
- Minimum 1 year of clinical experience; home health experience preferred
- Reliable transportation and valid driver’s license for patient visits
- Strong time management and organizational skills
Education
- Doctor of Physical Therapy (DPT) or equivalent — graduate of an accredited Physical Therapy program

- Therapeutic exercises and gait training
- Balance training and manual therapy techniques
- Developing and teaching home exercise programs
- Patient education and safety techniques
- Effective communication with interdisciplinary teams
