Roles & Responsibilities

• Conduct thorough patient assessments and document findings accurately in Epic
• Develop and implement individualized plans of care focused on recovery and function
• Provide gait training, ADL training, and use of assistive devices to maximize independence
• Deliver rehabilitation modalities such as cryotherapy and moist heat when appropriate
• Perform wheelchair assessments, prosthetic training, and safe transfer techniques including Hoyer lift use
• Reassess patients regularly, document progress, and coordinate discharge planning
• Collaborate with interdisciplinary teams including respiratory, pharmacy, radiology, and social services to optimize outcomes
• Float between acute care and acute rehab units as needed and respond to on-call requests

Qualifications

• Minimum 1 year of acute care hospital experience (explicitly noted)
• Active state physical therapy license required (pending licenses are not accepted)
• BLS certification preferred
• Experience charting in Epic is required
• Comfortable floating between like units and responding to on-call duties
• Trauma Level II experience preferred but not required

Knowledge & Skills

• Epic charting and accurate documentation practices
• Patient assessment, plan of care development, and reassessments
• Gait training, ADL training, and use of adaptive devices
• Prosthetic training and wheelchair assessment skills
• Hands-on experience with modalities such as cryotherapy and moist heat
• Safe lifting and transfer techniques including Hoyer lift and sit-to-stand devices
• Familiarity with billing populations: Medicare A, Medicare B, Medicaid, and commercial
• Ability to work with adult and geriatric patient populations and neurological, cardiopulmonary, oncology, and orthopedic cases
