Roles & Responsibilities

• Establish collaborative practice initiatives with oncologists to optimize chemotherapy and supportive care for inpatients and outpatients
• Design and deliver staff development and education for pharmacy, nursing, and medical teams
• Act as the go-to drug information expert for the oncology service line and ensure EHR care plans align with national guidelines
• Round with Oncology Fellows, Nurse Practitioners, and Physicians to recommend medication adjustments and monitor outcomes
• Provide patient-specific education using the teach-back method for oral oncolytics and IV chemotherapy
• Review and maintain oncology care plans in the Electronic Health Record for accuracy and guideline adherence
• Identify and resolve medication-related problems using clinical data and lab results
• Precept pharmacy residents and students, helping to train the next generation of oncology clinicians
• Contribute to Oncology Service Line meetings to improve safety, efficacy, and quality of cancer care
• Promote rational, safe, and cost-effective drug therapy with a focus on cost avoidance and savings
• Perform other duties within the scope of the role to support excellent patient care

Qualifications

• Doctor of Pharmacy (PharmD) required
• Licensed Pharmacist in good standing
• Board Certified Oncology Pharmacist (BCOP) required
• Minimum of three years of hospital experience with clinical pharmacy training
• Minimum of three years of oncology practice experience
• Completion of PGY-1 residency or equivalent clinical experience, or PGY-2 Oncology residency preferred
• Experience precepting residents and students is highly valued
• Comfortable reviewing and updating EHR care plans and providing drug information

Education

• Doctorate Degree in Pharmacy (PharmD)
• Post-graduate training: PGY-1 residency or PGY-2 Oncology residency preferred

Knowledge & Skills

• Deep knowledge of oncology pharmacotherapy and chemotherapy management
• Skilled at EHR review and aligning care plans with national guidelines
• Strong drug information and medication safety expertise
• Experience with supportive care measures and patient education (teach-back)
• Able to perform interdisciplinary rounding and communicate recommendations clearly
• Familiarity with cost avoidance analysis and promoting cost-effective therapy
• Ability to mentor fellows, residents, and students
