Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services to home health and hospice patients
- Help patients recover from surgeries, falls, and other conditions in the comfort of their homes
- Travel within a 60-mile radius around Valley City to provide care where it's needed most
- Document patient progress using the HomeCare/HomeBase EMR system
- Collaborate with interdisciplinary teams to support holistic patient recovery
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification required
- At least 1 year of previous home health experience
- Valid driver's license and auto insurance
- Strong PT-related clinical skills
Knowledge & Skills
- Home health and hospice patient care expertise
- Comfortable working independently in patients' homes
- Familiarity with HomeCare/HomeBase EMR
- Ability to manage a 5-7 patient daily caseload
- Knowledge of common diagnoses such as post-surgical recovery and fall prevention
