Roles & Responsibilities
- Provide compassionate physical therapy services directly in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs and create personalized care plans
- Guide patients and families through exercises and the safe use of therapeutic equipment
- Monitor progress and communicate updates to physicians while celebrating every milestone
- Collaborate with home health aides and coordinate additional support services when needed
- Document clinical notes and progress summaries that truly reflect patient achievements
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Valid driver's license and reliable transportation for home visits
