Roles & Responsibilities

• Evaluate patients to determine functional abilities and occupational needs
• Develop individualized, meaningful treatment plans that promote independence
• Implement therapeutic interventions to improve ADLs, strength, and coordination
• Collaborate with nurses, PTs, SLPs and the interdisciplinary team to drive patient-centered care
• Document assessments, progress notes and discharge plans accurately and on time
• Educate patients and caregivers on home programs, adaptive equipment and safety strategies
• Contribute to quality improvement initiatives and maintain regulatory compliance

Qualifications

• Licensed Occupational Therapist in New Hampshire (or COTA credential if applying as a Certified Occupational Therapy Assistant)
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Ability to work the scheduled day shift (8:00 AM - 4:30 PM) and start on the assignment start date
• Strong clinical reasoning, documentation skills and patient-centered approach
• Reliable, professional, and ready to collaborate within an interdisciplinary rehab team

Knowledge & Skills

• Skilled in OT assessment and intervention for functional independence
• Experience with ADL training, adaptive equipment and caregiver education
• Proficient with timely clinical documentation and care planning
• Strong verbal and written communication with patients and team members
• Good time management, prioritization, and safe patient handling skills
