Hours per Week: 36
Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide compassionate, hands-on care to residents in a supportive long-term care setting
- Collaborate with the care team to support residents with CHF, COPD, Alzheimer's, Parkinson's, and diabetes
- Administer medications with minimal IV involvement
- Maintain accurate documentation using PointClickCare EMR
- Foster a warm, home-like environment that promotes resident comfort and well-being
Qualifications
- Active LPN/LVN license in Minnesota
- Current BLS certification
- Strong teamwork and work ethic
- Ability to work every other weekend and holiday
Knowledge & Skills
- PointClickCare EMR experience
- Understanding of common long-term care diagnoses
- Ability to work collaboratively in a small, close-knit team
- Commitment to providing personalized, resident-centered care
