Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, 10:00 PM - 6:30 AM, every other weekend and holiday

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents in a supportive long-term care setting
Collaborate with the care team to support residents with CHF, COPD, Alzheimer's, Parkinson's, and diabetes
Administer medications with minimal IV involvement
Maintain accurate documentation using PointClickCare EMR
Foster a warm, home-like environment that promotes resident comfort and well-being

Qualifications

Active LPN/LVN license in Minnesota
Current BLS certification
Strong teamwork and work ethic
Ability to work every other weekend and holiday

Knowledge & Skills

PointClickCare EMR experience
Understanding of common long-term care diagnoses
Ability to work collaboratively in a small, close-knit team
Commitment to providing personalized, resident-centered care
