Roles & Responsibilities

• Administer medications and treatments as prescribed
• Monitor patients' vital signs and report changes to nursing staff
• Provide compassionate bedside care and support daily living activities
• Document patient care accurately in medical records
• Collaborate with RNs and physicians to ensure optimal patient outcomes
• Educate patients and families on care plans and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in Vermont
• Current BLS certification
• Strong clinical assessment and communication skills
• Ability to work night shifts and every weekend
• Experience in long-term care or skilled nursing preferred
• Must have own transportation

Knowledge & Skills

• Medication administration and documentation
• Patient care planning and implementation
• Wound care and basic life support
• EMR/EHR proficiency
• Excellent interpersonal and teamwork skills
• Attention to detail and critical thinking
