Roles & Responsibilities

• Provide compassionate direct patient care to residents and patients
• Administer medications and treatments as ordered by physicians
• Monitor and document vital signs, symptoms, and patient progress
• Collaborate with nurses, doctors, and other healthcare professionals to ensure quality care
• Assist with daily living activities and promote patient comfort
• Educate patients and families on health management and care plans
• Maintain accurate and timely medical records

Qualifications

• Active Licensed Practical Nurse (LPN) license in Connecticut
• Current BLS certification (or ability to obtain upon hire)
• Prior LPN experience in a skilled nursing or long-term care setting is a plus, but new graduates are welcome to apply
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Knowledge & Skills

• Knowledge of nursing protocols and patient care standards
• Proficiency in medication administration and dosage calculations
• Skill in using electronic health records (EHR) systems
• Ability to prioritize tasks and respond quickly in emergency situations
• Compassionate approach to patient care
• Strong attention to detail and documentation accuracy
