Roles & Responsibilities
- Provide compassionate, hands-on nursing care to patients at the Lebanon center
- Administer medications and treatments safely and in line with facility policies
- Assess patient conditions, recognize changes, and escalate to the care team
- Document patient care accurately and timely in the medical record
- Collaborate with RNs and interdisciplinary staff to support continuity of care
- Educate patients and families with clear, respectful communication
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Hampshire or eligible for licensure
- Graduate of an accredited practical nursing program
- Current BLS/CPR certification
- Previous LPN clinical experience preferred (long-term care or per diem experience a plus)
- Strong communication, teamwork, and time-management skills
Education
- Diploma or certificate from an accredited practical nursing program
Knowledge & Skills
- Medication administration and accurate documentation
- Patient assessment and basic nursing interventions
- Familiarity with electronic health records and charting
- Ability to prioritize tasks and manage time during 12+ hour shifts
- Professional bedside manner and effective communication with patients and families
