Roles & Responsibilities

• Provide compassionate, direct patient care to assigned residents and patients
• Administer medications and treatments in accordance with facility policy and provider orders
• Perform accurate assessments and monitor patient condition, reporting changes promptly
• Document care clearly and timely in the patient record using facility systems
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Support patient and family education to promote recovery and safe transitions

Qualifications

• Active LPN license in New Hampshire required
• Graduate of an accredited practical nursing program
• Current BLS (Basic Life Support) certification required or must obtain upon hire
• Previous LPN experience preferred but new graduates from LPN programs are welcome to apply
• Strong communication skills and ability to work well in a team-oriented environment

Education

• Completion of an accredited Licensed Practical Nurse (LPN) program

Knowledge & Skills

• Solid knowledge of medication administration and basic nursing procedures
• Comfortable performing patient assessments and monitoring vital signs
• Ability to document accurately and use electronic health record systems
• Good clinical judgment, time management, and prioritization skills
• Compassionate bedside manner and strong patient advocacy
