Roles & Responsibilities

• Conduct comprehensive psychosocial assessments to develop a deep understanding of each patient's unique needs.
• Create and implement individualized treatment plans in collaboration with the interdisciplinary care team.
• Facilitate therapeutic patient groups that support recovery and enhance coping skills.
• Coordinate discharge planning to ensure smooth transitions to community resources and support systems.
• Maintain accurate, timely, and thorough documentation of all clinical interactions and interventions.
• Advocate for patients and their families, providing education and emotional support throughout the treatment process.

Qualifications

• Active LCSW license in California (required)
• Master's degree in Social Work from an accredited program
• Minimum 1 year of hospital experience (strongly preferred)
• Inpatient psychiatric and/or geriatric psychiatry experience (preferred)
• Must be registered to run CA fingerprint background checks
• Must submit facility-specific skills checklist prior to submission

Education

• Master's degree in Social Work (MSW)

• Psychosocial assessment and evaluation
• Treatment planning and implementation
• Discharge planning and coordination
• Clinical documentation and record-keeping
• Group facilitation and patient education
• Knowledge of geriatric psychiatry and inpatient psychiatric care
• Excellent communication and interpersonal skills
• Ability to work collaboratively within a multidisciplinary team
