Roles & Responsibilities

• Provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
• Perform evaluations, reassessments, treatments, and clinical services within your scope of practice
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team
• Apply evidence-based interventions to improve function, independence, and quality of life
• Educate residents, families, and caregivers on treatment plans and progress
• Maintain accurate, timely documentation per facility, state, and federal guidelines

Qualifications

• Minimum two years of experience in SNF/ICF setting preferred
• Current Basic Life Support (BLS) certification
• Active State of Hawaii license in good standing for your discipline

Knowledge & Skills

• Skilled in rehabilitation therapy for long-term care
• Proficient with EMR systems: Net Health Rehab Optima and Epic
• Strong communication and teamwork abilities
• Ability to adapt to floating between units or facilities as needed
