Roles & Responsibilities

• Coordinate non-clinical behavioral health services and support program workflows
• Collaborate with interdisciplinary teams to ensure smooth patient transitions and referrals
• Document encounters and maintain accurate records in compliance with privacy standards
• Serve as a point of contact for patients, families, and community partners with empathy and clarity
• Support program operations by tracking outcomes, preparing reports, and identifying improvement opportunities
• Facilitate communication between clinical staff and administrative teams to remove barriers to care
• Assist with care coordination tasks such as appointment scheduling, resource linkage, and follow-up
• Participate in team meetings and contribute ideas to enhance patient experience and program effectiveness

Qualifications

• Experience working in behavioral health, care coordination, case management, or a related non-clinical role
• Familiarity with behavioral health workflows and documentation best practices
• Strong communication and interpersonal skills; ability to engage with diverse populations
• Comfort using electronic health records and basic data reporting tools
• Preferred: certification or training in behavioral health topics, case management, or care coordination
• Preferred: prior experience in a hospital, community mental health, or integrated care setting

Education

• High school diploma or GED required
• Associate or Bachelor’s degree in psychology, social work, public health, or related field preferred

Knowledge & Skills

• Knowledge of behavioral health systems, community resources, and referral pathways
• Proficiency with EHR systems and accurate clinical documentation
• Excellent organizational skills and attention to detail
• Ability to prioritize tasks and manage multiple coordination activities
• Strong customer service and conflict-resolution skills
• Basic data literacy for tracking outcomes and preparing reports
