Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, 8-hour day shifts

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a daily caseload of palliative care patients across multiple hospital units
Conduct comprehensive patient assessments and develop individualized care plans
Lead meaningful goals of care discussions with patients and families
Provide education on disease processes, advance directives, and POLST documentation
Facilitate family meetings and offer compassionate end-of-life support
Collaborate with physicians and the interdisciplinary team during daily rounds
Review patient charts and coordinate care to meet palliative screening requirements

Qualifications

Active California RN license required
2-5 years of recent palliative care or hospice experience
BLS certification required
ELNEC certification preferred
Strong acute care clinical thinking and communication skills
ICU or oncology experience is a plus

Knowledge & Skills

Cerner EMR proficiency
Excellent organizational and time management abilities
Compassionate approach to serving patients and families
Strong interpersonal skills for family meetings and care coordination
Flexibility to work across multiple hospital units and floors
