Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, 8-hour day shifts

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a meaningful caseload of 5-6 new patients and 3-4 follow-ups daily
Lead compassionate goals-of-care discussions with patients and families
Educate patients on disease processes, advance directives, and POLSTs
Facilitate family meetings and provide end-of-life support
Collaborate during daily rounds with physicians and the interdisciplinary team
Review patient charts and assess needs across multiple hospital units

Qualifications

Active California RN license
BLS certification required
ELNEC certification preferred
2-5 years recent palliative or hospice experience
Strong acute care background with excellent clinical thinking
ICU or oncology experience is a plus

Knowledge & Skills

Cerner EMR proficiency
Strong communication and organizational abilities
Flexibility to serve patients throughout the medical center
Compassionate approach to end-of-life care discussions
Ability to build trust with patients, families, and care teams
