Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with on-call rotation for evenings and weekends

Hours per Week: 40

Candidate Pay Rate: $62 per hour

Roles & Responsibilities

Conduct comprehensive initial assessments of clients and families to determine home health needs
Develop and regularly update individualized care plans based on patient diagnosis and changing needs
Administer medications and treatments as prescribed while monitoring patient response
Provide health care instructions and education to clients and families
Coordinate care as Case Manager and ensure all equipment and services are in place
Supervise and evaluate home health aide care to maintain quality standards
Communicate with physicians regarding patient needs and obtain necessary orders
Prepare clinical documentation and update patient records every sixty days

Qualifications

Current Registered Nurse license in North Dakota
BLS certification required
Home Health and Hospice experience required
Valid driver's license and auto insurance
Experience working with adults, geriatrics, and pediatric patients
