Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's strength, mobility, gait, and rehabilitation potential
Create and update individualized care plans in collaboration with physicians and families
Educate patients and caregivers on therapeutic exercises and equipment use
Monitor progress and communicate changes in condition to the care team
Coordinate with home health aides and other professionals to support rehabilitation goals
Document clinical findings and progress summaries to support continuity of care

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization
Ability to work independently while thriving as part of a collaborative team

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and rehabilitation planning
Knowledge of Title 22, Medicare, Medi-Cal, and OSHA regulations
Comfortable traveling between patient homes throughout Yolo County
