Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every other weekend rotation

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate comprehensive care plans for 15-20 patients daily across all hospital units
Complete initial assessments and utilization reviews using Interqual criteria
Develop discharge plans that address the unique needs of all payers, uninsured, and homeless populations
Collaborate with hospitalists, intensivists, and interdisciplinary teams to ensure seamless patient transitions
Document efficiently in Cerner while managing competing priorities throughout each shift
Partner with patients and families to create realistic, patient-centered discharge strategies

Qualifications

Active California RN license required
Current BLS certification
Recent experience with Dignity Health facilities strongly preferred
Proficiency with Interqual criteria and Cerner EMR system
Strong background in both utilization review and discharge planning
Excellent communication and collaboration skills with the ability to adapt quickly

Knowledge & Skills

Case Management and Care Coordination expertise

Discharge Planning across diverse patient populations
Cerner electronic medical record documentation
Interdisciplinary collaboration and teamwork
Effective communication and daily reprioritization abilities
