Location: San Rafael, CA

Contract Type: Contract

Shift Type: Full-Time; schedule to be determined (typical SNF day shifts — exact start/end times and days to be confirmed with the facility)

Hours per Week: 40

Candidate Pay Rate: $70 - $75 per hour

Roles & Responsibilities

Evaluate and assess patients’ physical mobility and functional abilities to create a clear baseline
Develop and implement individualized treatment plans focused on meaningful recovery goals
Deliver therapeutic exercises, mobility training, and rehabilitation programs with compassion and clinical rigor
Monitor patient progress, adjust treatment plans as needed, and document outcomes
Maintain accurate, timely documentation to support continuity of care
Collaborate with nursing, OT, speech therapy, and other interdisciplinary team members to deliver coordinated care
Educate patients and families on recovery strategies and safe mobility techniques

Qualifications

Active California Physical Therapist (PT) license required
Current BLS certification required
Previous SNF or rehabilitation experience preferred
Strong clinical judgment and excellent communication skills

Knowledge & Skills

Patient assessment and functional mobility evaluation
Individualized treatment planning and goal setting
Therapeutic exercise and rehabilitation program implementation
Clinical documentation and charting best practices
Interdisciplinary collaboration and patient-centered care
Strong verbal and written communication skills
