Roles & Responsibilities

• Provide compassionate physical therapy services to residents in a skilled nursing facility, helping them regain mobility and independence.
• Develop individualized treatment plans based on patient assessments and goals.
• Implement therapeutic exercises and modalities to improve strength, balance, and coordination.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document patient progress and adjust treatment plans as needed.
• Educate patients and families on home exercise programs and safety.

Qualifications

• Current California Physical Therapist (PT) license
• Doctor of Physical Therapy (DPT) degree
• New Grads welcome to apply
• Experience in SNF or rehabilitation settings is a plus, but not required
• Ability to pass a background check and drug screening

Education

• Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal skills
• Knowledge of CPT codes and documentation standards
• Ability to work independently and as part of a team
• Proficiency in electronic medical records (EMR)
