Roles & Responsibilities

• Perform comprehensive assessments to develop personalized treatment plans for residents
• Implement therapeutic exercises and interventions to improve mobility, strength, and independence
• Collaborate with the interdisciplinary care team to optimize each patient's functional outcomes
• Educate patients and their families on home exercise programs and fall prevention strategies
• Document patient progress accurately and timely to ensure continuity of care
• Adapt treatment approaches to meet the unique physical and cognitive needs of each resident

Qualifications

• Active Physical Therapist license in Minnesota, or eligibility to obtain one
• New graduates are welcome to apply
• DPT degree or equivalent
• Experience in skilled nursing or geriatric rehab is a plus, but not required
• Strong clinical reasoning and communication skills

Knowledge & Skills

• Knowledge of evidence-based physical therapy practices
• Skill in using electronic health records
• Ability to work independently and as part of a collaborative team
• Excellent interpersonal and patient education skills
• Compassionate and patient-centered approach to care
• Time management and organizational skills
