Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across your assigned region
Help patients recover from orthopedic injuries, strokes, joint replacements, and spinal conditions
Complete OASIS assessments to guide individualized care plans
Collaborate with PTAs, OTAs, and rehab aides to maximize patient outcomes
Document care using Cerner EMR and ensure compliance with home health regulations

Qualifications

Active Physical Therapist license in California
BLS certification required
At least one year of clinical experience as a Physical Therapist
Prior experience with OASIS documentation
Home health experience is a plus but not required

Knowledge & Skills

Strong assessment and treatment planning skills for diverse home health diagnoses
Comfortable working independently in patients' homes
Proficient with Cerner EMR and OASIS documentation
Excellent communication and time-management abilities
Flexible and adaptable to varying patient needs and schedules
