Roles & Responsibilities

• Deliver high-quality physical therapy services to residents under the direction of a licensed Physical Therapist
• Implement individualized treatment plans to improve mobility and functional independence
• Document patient progress accurately and in a timely manner
• Educate residents and families on therapeutic exercises and home safety
• Collaborate with the interdisciplinary care team to optimize patient outcomes
• Adapt treatment techniques based on patient response and progress

Qualifications

• Active Physical Therapist Assistant (PTA) license in California or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• New graduates are welcome and encouraged to apply
• Previous Skilled Nursing Facility (SNF) experience is a plus but not required
• Current BLS/CPR certification preferred
• Strong patient assessment, documentation, and communication skills

Knowledge & Skills

• Knowledge of physical therapy modalities and therapeutic exercise techniques
• Ability to adapt to changing patient needs and conditions
• Excellent interpersonal and patient-teaching skills
• Proficient in EMR documentation systems
• Understanding of safety and infection control protocols
• Compassionate, patient-centered approach to care
