Contract Type: Contract

Shift Type: Rotating shifts, 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, or 10:00 PM - 6:30 AM with cycle scheduling, every other weekend and holiday

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents in a supportive long-term care setting
Collaborate with the charge nurse and care team to ensure smooth daily operations
Monitor and support residents with conditions such as CHF, COPD, Alzheimer's, Parkinson's, and diabetes
Administer minimal IV medications safely and effectively
Document care accurately using PointClickCare EMR
Foster meaningful connections in a home-like environment

Qualifications

Active LPN/LVN license in Minnesota
Current BLS certification required
Strong teamwork and work ethic
Experience in long-term care preferred

Knowledge & Skills

PointClickCare EMR proficiency
Understanding of common long-term care diagnoses
Ability to work collaboratively with nursing staff
Attention to detail in medication administration
