Roles & Responsibilities

Provide direct, compassionate nursing care to residents/patients as assigned
Administer medications and treatments safely and accurately
Monitor and record vital signs, reporting changes promptly to the care team
Collaborate with nurses, providers and support staff to execute care plans
Maintain clear, timely documentation in the patient record
Educate residents and families with clarity, respect, and empathy

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willing to obtain upon hire)
Completion of an accredited practical nursing program
Up-to-date immunizations and ability to meet facility health requirements
Strong communication skills and a team-first attitude
Prior experience in long-term care or similar setting is a plus

Education

Graduation from an accredited Licensed Practical Nurse (LPN) program
High school diploma or GED

Knowledge & Skills

Medication administration and safe documentation practices
Basic assessment skills: vitals, skin integrity, mobility and cognition
Familiarity with infection control and patient safety protocols
Comfortable using electronic medical records and charting systems
Ability to prioritize tasks in a fast-paced clinical environment
Excellent interpersonal skills for patient and family interactions
