Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents throughout your shift
• Monitor vital signs, observe changes in condition and report findings promptly
• Administer medications and treatments accurately while following care plans
• Document care thoroughly and on time using PointClickCare (PCC)
• Collaborate with RNs, CNAs and physicians to keep every resident safe and comfortable
• Support residents and families with kindness, patience and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) license in Vermont
• PointClickCare (PCC) experience required, you will hit the ground running
• Comfortable working in a skilled nursing or long-term care setting
• Current COVID-19 vaccination or a documented religious or medical exemption
• Able to bring state or federal issued identification and wear a photo badge at all times
• Local candidates only, this role is not open to travelers
• Reliable, warm and ready to jump in with a team that needs you

Knowledge & Skills

• Strong clinical assessment and observation skills
• Medication administration and treatment documentation
• Electronic health record proficiency, especially PointClickCare (PCC)
• Clear written and verbal communication with residents, families and staff
• Time management that keeps care on schedule during busy shifts
• Team-first attitude with a genuine heart for patient care
