Roles & Responsibilities

• Deliver compassionate bedside care that makes a real difference in residents' daily lives
• Monitor vital signs, observe changes, and report updates to the care team
• Administer medications and treatments as ordered, keeping accurate documentation
• Support residents with mobility, personal care, and daily living activities
• Collaborate with RNs, therapists, and physicians to keep care plans on track
• Chart thoroughly and accurately in the facility's electronic medical record
• Respond quickly and calmly to changes in condition and urgent needs
• Educate residents and families with clear, reassuring guidance

Qualifications

• Active LPN license in the state of Vermont
• Current BLS certification
• Completion of the original or updated COVID-19 vaccine, or a documented religious or medical exemption
• Willingness to bring a state or federal issued ID and wear a photo badge at all times
• Prior experience in skilled nursing, rehab, or long-term care is a plus
• Strong communication skills and a genuine passion for patient care
• Ability to commit to a 13-week local contract with weekend availability
