Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long-term care setting
• Administer medications and treatments accurately and safely according to orders
• Monitor, assess and document resident conditions, vital signs, and changes in status
• Collaborate with nursing staff and interdisciplinary team to develop and follow care plans
• Support residents with activities of daily living while promoting dignity and independence
• Communicate clearly with residents’ families and the care team to ensure continuity of care
• Maintain accurate and timely electronic and paper documentation in line with facility policies

Qualifications

• Active LPN license in Pennsylvania (required)
• Completion of an accredited LPN/LVN program (diploma or certificate)
• Current BLS/CPR certification (or willing to obtain upon hire)
• Ability to pass standard background checks and meet immunization requirements
• Strong communication skills, empathy, and a team-oriented approach
• Prior experience in long-term care or a similar clinical setting is a plus

Education

• Completion of an accredited Practical Nursing program (LPN/LVN diploma or certificate)
• High school diploma or GED

Knowledge & Skills

• Clinical assessment and basic nursing care techniques
• Medication administration and safe documentation practices
• Infection control and standard precautions
• Familiarity with electronic health record (EHR) charting
• Ability to prioritize tasks in a busy clinical environment
• Effective communication with residents, families, and interdisciplinary teams
