Roles & Responsibilities

• Implement occupational therapy treatment plans to help residents regain functional independence
• Work closely with the supervising OT and interdisciplinary team to coordinate patient-centered care
• Complete timely, accurate documentation in Net Health according to facility standards
• Ensure compliance with Medicare guidelines, facility policies, and regulatory requirements
• Deliver compassionate care for short-term and long-term residents, adapting plans as needs change
• Communicate progress, barriers, and discharge planning details with team members and families

Qualifications

• Active OTA (Certified Occupational Therapy Assistant) license required
• Skilled Nursing Facility (SNF) experience preferred; mentorship available for clinicians with 0-1 year experience
• Strong documentation skills and familiarity with Net Health (or quick willingness to learn)
• Excellent time-management skills and a flexible, collaborative mindset
• Ability to work effectively within an interdisciplinary team and follow established care plans
