Shift Type: Shift 3 (NOC), 11:00 PM - 7:30 AM, weekends required

Hours per Week: 40

Candidate Pay Rate: $42 per hour

Roles & Responsibilities

  • Provide direct nursing care to residents in a skilled nursing facility, ensuring their comfort and safety
  • Administer medications, injections, and treatments as prescribed, monitoring for effectiveness
  • Collaborate with RNs and the interdisciplinary care team to create and update individualized care plans
  • Document patient assessments, observations, and interventions accurately in the medical record
  • Assist with daily living activities and promote resident independence and dignity
  • Educate residents and families on care plans, medications, and health maintenance

Qualifications

  • Active Licensed Practical Nurse (LPN) license in Vermont or a compact NLC state
  • At least 1 year of recent LPN experience in long-term care or skilled nursing (preferred)
  • Current BLS/CPR certification required
  • Ability to pass background checks and drug screening
  • Comfortable working night shifts and weekends

Knowledge & Skills

  • Strong clinical assessment and critical thinking skills
  • Proficiency in medication administration and treatment protocols
  • Excellent communication and interpersonal skills
  • Familiarity with electronic health records (EHR) systems
  • Ability to work effectively in a fast-paced, team-oriented environment
  • Compassionate, resident-centered approach to care
