Roles & Responsibilities

• Administer medications and treatments accurately and safely
• Monitor and report patient vital signs and changes in condition
• Provide compassionate care with daily living activities and wound care
• Collaborate with RNs, physicians, and the care team to ensure quality outcomes
• Document patient care thoroughly and timely
• Educate patients and families on health management and discharge plans

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification
• Experience in long-term care, skilled nursing, or similar setting preferred
• Strong clinical judgment and assessment abilities
• Flexibility to work various shifts as needed

Education

• Graduate of an accredited practical nursing program

Knowledge & Skills

• Knowledge of nursing principles and evidence-based practices
• Proficiency in medication administration and patient monitoring
• Excellent communication and interpersonal skills
• A caring, patient-centered approach
• Ability to adapt quickly and work effectively in a dynamic environment
