Roles & Responsibilities

• Provide direct patient care including medication administration, wound care, and treatments
• Monitor and document patient vital signs and overall condition
• Collaborate with registered nurses and the healthcare team to deliver high-quality care
• Assist residents with daily activities and promote comfort and dignity
• Communicate effectively with patients, families, and team members
• Ensure accurate and timely documentation in accordance with facility policies

Qualifications

• Current LPN license in the state of Pennsylvania
• CPR/BLS certification required
• Long-term care or skilled nursing experience preferred
• Strong clinical skills and attention to detail
• Ability to work independently and as part of a team

Knowledge & Skills

• Knowledge of LPN scope of practice and nursing fundamentals
• Medication administration and basic life support skills
• Strong communication and interpersonal abilities
• Familiarity with electronic health records (EHR) is a plus
• Compassionate, patient-centered approach to care
